Emergency Service Contract

I, as the owner or authorized representative, grant authorization to Michigan Home Pro's, Inc. (MHP) to proceed with the necessary actions to restore, rebuild, clean, deodorize, or provide emergency services for the building structure and/or contents. This authorization is in accordance with the directives of the owner or insurance representative concerning the loss or work involved. The undersigned individual represents either the owner or possesses the necessary authority to sign on their behalf. The owner acknowledges the responsibility to compensate MHP for all labor, materials, and equipment employed during the mobilization, demobilization, and execution of the required tasks. The pricing details have been provided and agreed upon as outlined below. I understand and agree that Michigan Home Pro's and its employees shall not be held liable for any damages that may arise as a result of their work.

In the event that a lump sum is not specified above, it is understood and agreed by the owner that a price will be determined at a later date. This price will be based on MHP's Time, Equipment, and Materials, or it will be determined using independent pricing established for insurance restoration purposes by Xactimate. The owner agrees to make payments directly to Michigan Home Pro's for the work performed, including any deductibles, depreciation, or expenses not covered by insurance. Prior to the commencement of work, the owner also agrees to remove any cash, jewelry, firearms, collectibles, or other valuable items from the premises. Furthermore, the owner requests that all payments related to MHP's work be made directly to MHP by their insurance provider. Additionally, the owner requests that MHP be added as a payee on all claim payments. In the event that this arrangement is not implemented, the owner assigns the insurance proceeds, to the extent that they pertain to MHP's work. Upon notifying the insurance company, it is expected that they will initiate contact within a timeframe ranging from 1 to 36 hours. It is important to inform MHP when the adjuster meeting is scheduled by contacting them at (517)939-9306 or (517)881-9344. MHP will make all necessary arrangements to accommodate the adjuster's schedule and ensure their presence during the scheduled meeting. Any adjustments required to our schedules will be promptly made by MHP.
